Eligibility

To be enrolled in the Home & Community Based Services (HCS), the person must have:

A Determination of Mental Retardation
Be Medicaid eligible
Received a HCS Waiver slot

 HCS Services Include
  • Residential Support—3-4 person group homes
  • Foster Care or Companion Living—Provided in a contracted family’s home
  • Supported Home Living—Assistance provided in an individual’s own home
  • Day Habilitation
  • Supported Employment
  • Case Management.
  • Counseling & Therapies
  • Nursing
  • Respite
  • Psychological Services
  • Dietary Services
  • Minor Home Modifications
  • Adaptive Equipment
  • Social Work Services
